All American Community Health Center is a nonprofit community clinic opening in Pomona, CA. We are seeking a compassionate and reliable Family Nurse Practitioner (FNP) to provide primary care services to patients in a community clinic setting. The FNP will evaluate and treat patients, perform routine primary care visits, support preventive care, manage basic chronic conditions, document care in the EHR/EMR system, and work closely with the clinic team to provide high-quality, patient-centered care. This is a great opportunity for a provider who wants to be part of a new nonprofit clinic from the beginning and help serve local families in Pomona and surrounding communities.
Responsibilities
- Provide primary care services to patients in a clinic setting
- Perform patient assessments, exams, diagnosis, and treatment planning
- Manage routine primary care visits, preventive care, and follow-up care
- Support care for common chronic conditions such as diabetes, hypertension, and asthma
- Order and review labs, imaging, and other diagnostic tests as appropriate
- Prescribe medications within scope of practice and applicable regulations
- Document patient visits accurately in the EHR/EMR system
- Educate patients on treatment plans, medications, prevention, and lifestyle changes
- Coordinate care with medical assistants, front desk staff, and outside providers
- Follow clinic policies, HIPAA guidelines, and quality standards
- Help support the growth and workflow development of a new community clinic
Qualifications
- Active California Nurse Practitioner license required
- Family Nurse Practitioner certification required
- Furnishing number required or strongly preferred
- DEA registration preferred
- Primary care, family medicine, or community clinic experience preferred
- Experience with Medi-Cal or underserved patient populations is a plus
- Bilingual English/Spanish preferred, but not required
- Strong communication and patient care skills
- Comfortable working in a new clinic environment
- Reliable, organized, and able to work as part of a team
- EHR/EMR experience preferred
