Community Wellness Resources
Nutrition resources can help patients ask practical questions about food access, chronic conditions, family meals, and community programs. All American Community Health Center serves patients and families in Pomona and nearby communities.
Nutrition support may include discussing family meals, blood pressure, diabetes, weight goals, food budget, cultural foods, and what changes feel realistic.
Healthy eating is harder when families face cost, transportation, housing, or work schedule barriers. Ask about community resources and referrals when available.
Patients with diabetes, high blood pressure, pregnancy questions, kidney disease, or digestive symptoms should ask a clinician before making major diet changes.
This page is for education and care planning only. It does not diagnose a condition or replace medical advice from a licensed clinician. For severe symptoms, chest pain, trouble breathing, signs of stroke, heavy bleeding, serious injury, or thoughts of self-harm, call 911 or go to the nearest emergency department.
