The Site is intended for adults age 18 and older. By using the Site, you represent that you meet this requirement. Parents and guardians may use the Site to schedule appointments and obtain information for minor patients.

3. No Clinician–Patient Relationship
Using the Site, submitting a contact or appointment form, or otherwise communicating with us through the Site does not create a clinician–patient relationship. A clinician–patient relationship is established only when you are accepted as a patient at our clinic.
4. Medical Emergencies
If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use the Site, our forms, email, or any other electronic communication for emergency or urgent care needs. We do not monitor these channels in real time.
5. Acceptable Use
